Medical forms as of March 2011
All Scouts and adults must have current medical forms on file to participate in troop activities. This form has three parts: A, B, and C.
Part A and Part C must be completed annually be all troop members. Part A is a detailed medical history, and Part C is a liability release form. For Scouts, Parts A and C can be filled out by the Scout’s parents. Adult Scouters can fill out Parts A and C themselves.
Part B of the medical form is required for events over 72 hours or strenuous activities, including summer camp, high adventure trips, and physically demanding service projects. Part B must be completed and signed by a certified and licensed health-care provider. In an addition to the old Class 3 forms, Part B now contains height and weight guidelines.
